Chennai, 31st October 2023 In a significant move to raise awareness about breast cancer and emphasize the importance of early detection, Apollo Proton Cancer Centres (APCC), the first and largest Proton Therapy Centre in South Asia and the Middle East, along with the Department of Social Welfare & Women Empowerment, Government of Tamil Nadu, released a Self-Breast Examination Chart and Animated Video. The unveiling of this pivotal initiative took place at Commissioner of Social Welfare office in Chennai. The awareness video and chart will be played in 2286 Primary Health Centres, 434 Child Development Project Offices, 38 District Social Welfare Offices, 38 District Project Offices and 38 Collectorate Offices and all Corporations.

The Chart was unveiled by Ms. P Geetha Jeevan, Hon’ble Minister for Social Welfare & Women Empowerment Department, Government of Tamil Nadu, along with Thiru. Dr Manjula Rao, Consultant – Breast Oncoplastic Surgery, highlighted the importance of early detection, saying, “Breast cancer is associated with excellent prognosis and survival when detected early. If detected before it spreads to the regional lymph nodes, 5-year survival outcomes are as high as 99%. Self-breast examination is a simple tool that women can use, to be mindful of their breast health and catch a breast tumour early. While the majority of breast lumps are not cancerous, it is important to get them shown to a doctor who can rule them out. Painless breast lump, spontaneous nipple discharge, nipple retraction, skin redness, dimpling, non-healing ulcer, etc. are some signs and symptoms to look out for.”

Dr. Prasad E, Senior Consultant – Medical Oncology, shed light on various methods available for early detection, stating, “Numerous methods are available for the early detection of breast cancer, with the goal of treating people on time. For women aged 40 and above, digital X-rays and mammograms are imperative. When there is a strong family history of cancer, early screening with ultrasounds, mammograms, and, in some instances, MRI scans are recommended to detect early lesions. The primary focus should always be on preventing breast cancer through lifestyle adjustments, even though the latest treatments offer exceptionally high cure rates.”

Everything you need to know – Male Breast Cancer

Breast cancer is a type of cancer that is most commonly seen among women; however, it does occur in men as well. This is because, although minimal in quantity, men do have breast tissue that has the potential to become malignant similarly to women, albeit much less commonly and with a known ratio of 1 male case to 100 in females and occurs in about 1% of all breast cancers. Male breast cancers generally account for 0.5% of all cancer diagnosis among men, but there has been a recent concern about the rising incidence in the past few decades.

Risk Factors for Male Breast Cancer:

Some of the most prevalent risk factors for breast cancer in men are:

1. Family History: Having a first- or second-degree relative with breast cancer increases a man’s risk of developing the disease, therefore family history is important.

2. Other possible triggers have also been found, and they include disorders with elevated oestrogen circulation, such as cryptorchidism, orchitis, and orchiectomy, obesity, marijuana usage, hepatic dysfunction, thyroid illness, and oestrogen-containing drugs.

3. Lastly, environmental factors have also been linked to a higher risk of breast cancer in men than in women. These factors include radiation therapy prior to cancer treatment, jobs involving exposure to or use of organic solvents like trichloroethylene, and occupations

What to look out for and when to consult:

Male patients most frequently complain of a painless mass in one breast, along with other possible symptoms such nipple retraction, nipple discharge, ulceration, and pain when they visit the clinic. Male gynecomastia masks the problem, however overall, male masses are simpler to appreciate than female ones because of the smaller breast size.

Test include a triple assessment to diagnose male breast cancer and includes the following:

1. Clinical evaluation

2. Ultrasonography or mammography.

3. Core biopsy

With a sensitivity score of about 92% and a specificity score of about 90%, mammography is one of the most successful diagnostic techniques; nevertheless, this modality may occasionally be limited because male breasts differ in size and volume. Because of this, the most accurate diagnosis for male breast cancer is provided by ultrasound-guided core biopsy, which is favoured over fine-needle aspiration.

Chances of Cure:

Male and female survival rates are comparable across stages. The overall 5-year survival rate for male breast cancer is between 40% to 65%, primarily due to delayed diagnosis. Stage I would be around 75–100%, Stage II would be approximately 50–80%, Stage III would be approximately 30–60%, and Stage IV would be approximately 20–30% when stratified.

Treatment

The same guidelines that apply to the treatment and management of a female patient with breast cancer also apply to male patients, and these guidelines would be:

· Resection of the tumor or mass, with adjuvant endocrine therapy, chemotherapy (CT), or radiation therapy (RT) administered thereafter

· A straightforward mastectomy performed in isolation for small lesions, and in conjunction with radiation therapy (RT) and chemotherapy (CT) for more advanced tumor stages

· Tamoxifen should be used as adjuvant treatment for a minimum of five years

· Tamoxifen is recommended above other treatments since the majority of metastatic male breast cancers are oestrogen receptor positive

Post-Treatment Care: Long-term monitoring is necessary because men are at very high risk for a second ipsilateral or contralateral breast lesion.

Screening: Routine screening of males with breast cancer is not established due to the rarity of the disease occurrence in the general male population. Yet, males should be educated on the potential risk factors discussed above and should be advised on genetic counselling if they have a family history of breast cancer (in both males and females) or other cancers that may be triggered by a BRCA gene, for example, an ovarian, pancreatic, testicular, or prostate cancers.

Final Note: Male breast cancer is uncommon, but if diagnosed early is curable. The mainstay of the treatment is excision and hormonal therapy, genetic counselling in case of family history, and consultation with a doctor in case of slightest suspicion increases the chance of cure.

26-30% of women come in a later stage for breast cancer treatment

Pune: Breast cancer is a widespread issue that affects women from all walks of life, including those living in rural areas. Without adequate knowledge or access to regular check-ups, women living in rural areas remain ignorant about simple practices like self-breast examinations that could potentially save lives. Women in rural areas should be encouraged to perform self-breast examinations at least once a month after turning 25. Timely detection of breast cancer and prompt treatment are key to saving the lives of women.

According to the Indian Council of Medical Research (ICMR), breast cancer in India varies from as low as 5 per 100,000 female populations per year in rural areas to 30 per 100,000 female populations per year in urban areas. When compared to urban areas, women in rural areas are unaware of breast cancer leading to loss of life due to ignorance.

Dr. Jaypalreddy Pogal, Surgical Oncologist, TGH-Onco Life Cancer Center, Talegaon says, “Breast cancer affects millions of women not only in India but globally, and continues to claim lives due to lack of awareness in rural areas. The disparity between urban and rural communities when it comes to access to information and healthcare services is striking. In rural areas, the limited availability of health facilities and resources hinders early detection and treatment for breast cancer. More than 80% of women living in rural areas are more likely to be unaware of symptoms such as lumps in their breasts, changes in breast size or shape, redness or thickening of the skin, nipple inversion or discharge, and sudden pain in the breast. They often land up in hospital in later stages when their condition worsens. This leads to poor prognosis of the cancer and higher morbidity and mortality rates. 26-30% of women who came in later stages. Fear of judgment or stigma acts as a significant barrier preventing women from seeking help or getting screened for this deadly disease. Many women are simply unaware of risk factors such as family history, obesity, or smoking that can increase the likelihood of developing breast cancer.”

Dr. Jaypalreddy Pogal added, “By organizing small workshops or health camps tailored specifically for these communities, information about breast cancer can be disseminated effectively. Promote early detection methods to help women understand the symptoms. Educating women about self-examination techniques and the importance of regular check-ups can significantly improve detection rates, and help with prompt treatments. Women in rural areas should be encouraged to perform self-breast examinations at least once a month after turning 25. Also, help women with training sessions, pamphlets, and educational videos on how to perform self-examinations at home.”

“Women who receive timely interventions, such as surgery or radiation therapy, have a higher chance of survival compared to those with late-stage diagnoses. When breast cancer is caught early, it can be managed with breast-conserving surgery. Breast-conserving surgery is surgery to remove the cancer as well as some surrounding normal tissue. Only the part of the breast containing the cancer is removed. How much breast is removed depends on where and how big the tumor is, as well as other factors. This surgery is also called a lumpectomy, quadrantectomy, partial mastectomy, or segmental mastectomy. By intervening in a timely manner, women can potentially avoid more extensive procedures and their associated complications. Mr Joykrishna Barik is a blind agriculturalist from Kanthi, West Bengal. He received his training in agriculture, gardening and cultivation from Ramakrishna Mission Blind Boys’ Academy. Later, he started a nursery business and named the nursery as ‘Ramkrishna Sarada Maa’ nursery. Mr Barik received a state award in 2016 and 2022. Being blind, he has taken the initiative of running a nursery and providing saplings to various organisations, thereby creating a unique commitment for creating a ‘greener tomorrow’.

Asansol Braille Academy (ABA), the School unit of the Asansol Prevention Of Blindness Society, is a school for the blind, residential & Free from all charges, successfully running for the last 19 years in Asansol. Presently 30 students, from Jharkhand to Durgapur Steel Township and neighbouring districts of
West Bengal are residing and taking education. It is not a government-aided school and is purely run by the NGO, APBS.

Ms Sangita Metya, a blind girl from Bengal has represented the Indian team for the ‘World Championship of Blind Football’ that was held at Birmingham in the UK in August. Ms Pratima Ghosh is blind since birth. A Nabadwip resident and the daughter of a daily labourer, she went through many hardships in life and now she is making Bengal proud.
