Hyderabad, 22nd September 2023— American Oncology Institute (AOI) Hyderabad successfully treated a 4-year-old girl through a groundbreaking Haploidentical stem cell transplantation, where her mother served as a donor. The young patient, named Deepika, initially sought medical help due to bleeding gums and frequent nosebleeds (epistaxis).

Her condition was further complicated by a drastically low platelet count (thrombocytopenia), which reached a critical level of 3000. Suspecting immune thrombocytopenia, Dr Ashok Kumar, Haemato-Oncologist and bone Marrow Transplant Physician at AOI initiated treatment with steroids. Upon further investigation, her bone marrow tests indicated amegakaryocytic thrombocytopenia, a rare congenital disorder where the bone marrow fails to produce sufficient platelets.

Dr Ashok Kumar, shared his insights, saying, “Congenital amegakaryocytic thrombocytopenia (CAMT) is a rare, inherited disorder characterized by a severely low number of megakaryocytes, a type of bone marrow cell that makes platelets that are important for clotting and preventing bleeding. Deepika’s case exemplifies the extraordinary resilience of young patients and the power of advanced medical interventions. Our collaborative approach, combined with her mother’s selfless contribution as a donor, paved the way for this heartening success.”

Advancing their diagnostic efforts, the medical team employed Next-Generation Sequencing (NGS) and identified a homozygous MPL mutation, a genetic alteration related to congenital a megakaryocytic thrombocytopenia. Given the complexity of the case, AOI Hyderabad’s experts decided on a curative approach—Haploidentical stem cell transplantation, using Deepika’s mother as the donor.

The peritransplant period proved difficult for Deepika, as she encountered complications such as seizures, hypertension, and others. However, the medical team managed these symptoms with proper medication and ensure her safety and recovery. After five months of rigorous medical care, Deepika made remarkable progress. Her platelet count improved significantly, exceeding 100,000, a positive sign of her body’s response to the treatment. As her health stabilized, her medications were carefully tapered, and she now relies on a single medication.

Chennai, September 21st, 2023: Gleneagles Global Health City a leading tertiary care hospital in Perumbakkam, Chennai has successfully performed India’s first breakthrough complex brain surgery of a 38-year-old man from Chennai. A team of expert Neurosurgeons effectively removed two tumors in Mirror Image fashion in both left and right side of the eloquent brain. Usually, this surgery is very risky in nature, because if complications occur the patient could turn blind. They went ahead and risks were explained to the patient and surgery done in timely manner to save the life of 38-year-old professional. The surgery was conducted in two-stages and took just over 10 hours in total.

38-year-old professional from Chennai had a rare incidence of having tumor on both the right and left side of the brain. The size of each tumor was of 5 x 7 cms and were located in the parieto-occipital area of the right and left side of the brain, the area concerned with vision. The professional had exhibited symptoms like recurrent headaches with visual seizures for the past three months.

Dr. Nigel P Symss and his team conducted several assessments and special MRI scans on the patient prior to the surgery. The patient was advised to undergo two-staged procedure called Right and Left Parieto Occipital Craniotomy and excision of space occupying lesion, which is considered as one of the high-risk procedures in brain surgery. The conduct of the surgery is dependent on precision, expertise and high-end technology, using Microneurosurgery, Navigation and Intraoperative Neuromonitoring. The patient was counselled and was prepared for this two-staged high-risk procedure. It was also informed that the delay in the removal of the tumors could result in worsening of vision and other serious complications.

The patient first underwent right parieto-occipital craniotomy and excision of right parieto occipital lesion. Post operative period were uneventful and he was discharged home after 5 days. He was prepared for second stage surgery conducted two weeks later and the left parieto occipital craniotomy and excision of left parieto occipital lesion was done. Post the staged procedures patient has recovered and he continues to have 80 to 90 percent of his vision in both eyes without complications and he was discharged after four days post the second successful surgery.

The Summit was attended by over a hundred orthopaedic surgeons from across Telangana including several young ortho professionals. The thrust of this year’s Conference is on teaching young orthopaedic surgeons the latest surgical techniques. Close to 50 Orthopaedic Procedures and techniques were imparted under one roof in a day, by renowned national and local faculty.

This exhaustive teaching exercise is aimed to guide young orthopaedic surgeons in honing their knowledge and aptitude regarding correct indications and techniques of surgery. Our conference over the years has received immense patronage from the delegates for the valuable insights they gain here, while helping multiple students in gaining precision, says Dr N. Somashekar Reddy. This Summit is focusing on treating joint problems without resorting to knee or hip replacement surgeries. The deliberations were on how to grow cartilage between the joints and provide relief to the patients who can be managed without joint replacement and in cases where joint replacement is mandatory, is there a difference in robotic and regular surgeries.

The Summit focussed on newer frontiers in orthopaedic care using technology, especially robotic and AI dependant treatments, to enable greater surgical precision, speedy patient recovery and reduce the burden of treatment cost on patients. Robotic Knee Replacement as a new revolution in joint replacement surgery aims at accuracy and better patient outcomes. The Seminar visualised tremendous potential in technology revolutionising orthopaedic treatment.

Though Robotic surgery is yet to be proven through long term results and research, it has definitely proved handy in cup positioning in Hip Replacement Surgery and also in severe deformity knee replacements. An aatmanibhar robot could be the answer to reduce costs for patients in India without compromising on precision, that could be a game changer with the treatment becoming affordable to more patients in India. We are in talks with technocrats and start-ups to develop a robot prototype for ortho care, says Dr Mithin Aachi. At this Summit, we are focusing on new technologies that have come in orthopaedics especially robotics, cartilage replacements, platelet rich plasma and multiple other new options.

Prof SS Mohanty said, the Summit discussed about joint preservation surgeries, especially among the young patients. At least 15% to 20% patients are not satisfied with the conventional knee replacement surgery, therefore robotic assisted surgery is enabling joint alignment and stability and enhancing patient satisfaction levels and providing immediate relief from pain. In young patients, we opt for alternate procedures like platelet rich plasma, cartilage transplantation, realignment of the bone in the joint, to reduce trauma and preserve the joint for the next 15 to 20 years before the patient needs to go for joint replacement.

Dr Debashish Chandra said, we discussed at the Summit about how to preserve the joints and avoid the knee or hip replacement for patients under 50 years of age. Young patients in the age group of 20 to 40 years, who damaged their cartilage and had no option earlier, can benefit from cartilage transplant and implantation, cartilage fillers etc., Such patients have multiple options to lead a better life, which are equally successful with alignment correction surgeries.

The Seminar was of the opinion that a senior experienced surgeon gives equal and parallel results to Robot, but it is handy in patients with extremely bad deformities. The delegates felt that Making In India perhaps will be the best way to cut costs of Robot manufacture and procurement.

New single stage techniques for Cartilage lesions is another area which was deliberated and is fast proving in short term results to be very reliable to prevent osteoarthritis in future. However long term results are very important before wd can generalise outcomes. Platelet

The experts felt Rich Plasma is a good and proven way to slow down many pathologies in the knee. However it was observed that the failures were due to deformity being severe, experts in joint pathology not being involved and proper indications not being followed. Osteotomies, minimal invasive fixations and angle corrections were the other new modalities in joint preservation, which were discussed at the Seminar.
