Mumbai, 5th February 2024: The number of cancer cases in the country is projected to go up from 14.6 lakh in 2022 to 15.7 lakh in 2025, according to the Indian Council of Medical Research-National Cancer Registry Programme (ICMR-NCRP)[1]. In India, there is a likelihood of one in nine individuals developing cancer during their lifetime. The Indian Journal of Medical Research[2] reports that lung cancer is the predominant site for males, while breast cancer leads among females. In the case of childhood cancers (0-14 years), lymphoid leukaemia holds the highest incidence, accounting for 29.2% in boys and 24.2% in girls. Projections indicate a 12.8% estimated increase in cancer cases by 2025 compared to the figures recorded in 2020.

Farida serves as an executive committee member of the Multiple Sclerosis Society of India- Hyderabad chapter (MSSI). This non-profit organisation works for the welfare of people affected by Multiple Sclerosis and is affiliated with MSIF (Multiple Sclerosis International Federation).

Multiple sclerosis (MS) is a chronic neurological disorder that affects the Central Nervous System [CNS]. It is a potentially disabling disease that is creating havoc in the lives of many families, informed Dr. Sudhir participating in a panel discussion that was organised before the launch of the book. The panel discussion was moderated by Hari Mohan, a first-class cricketer, and former Ranji Cricket Player.

MS—Multiple Sclerosis patients no longer need to be called so, I call MS Patients Most Special patients, said Dr Sudhir Kumar, a renowned Neurologist currently working in Apollo Hospitals said. Speaking about the book, Dr Sudhir said, this book has so many things which many doctors are not aware of nor any neurological books talk about. The treatment goes beyond medication. And those things which go beyond treatment are found in this book he said.

Meena Gupta said MS patients are not covered under Insurance. So the theme for this year’s MS Day was #InsureMyMS. She sought support for MS patients in getting disability certificates from the government without much hassle.

Though its prevalence was higher in the West, now there is a rise in these cases in India. Eight to nine people per one lakh population is estimated to be suffering from MS. India has a disease burden of roughly 2.5 lakh. The number is estimated to be higher than stated, as not many people are diagnosed due to the lack of awareness. Manish Sisodia’s wife is a well-known case of multiple sclerosis currently. The Telugu states have around 200 MS patients registered with the MS Society of Hyderabad.

Farida Raj, a Mumbaikar relocated to Hyderabad (after marriage with Vijay Mohan Raj, a Ranji Cricketer and contemporary of Kapil Dev) and made Hyderabad her home for the past 41 years.

Farida has done a lot of work in the area of Specific Learning Difficulties (SLDs). In the past, she authored three insightful books, BREAKING THROUGH, UNDERSTANDING LEARNING DIFFICULTIES and STRUGGLING MINDS, on the subject, aiming to empower both teachers and parents in supporting their children’s learning journeys. Breakthrough Surgery: 27-Day-Old Infant Cured of Rare Lung issue

Bangalore, 31st January 2024 – In a recent medical marvel, doctors at Aster RV Hospital, JP Nagar, Bengaluru successfully treated a 27-day-old male baby suffering from congenital lobar emphysema, an exceedingly rare condition causing abnormal lung lobe expansion, leading to critical complications in breathing and oxygenation.

27 day-old male baby was brought to the hospital with complaints of breathing difficulty. Parents had noticed the child had discomfort in breathing since birth. The child was rushed to the hospital on observing continuous difficulty in breathing. On examination, it was observed that the child had reduced air entry on the right side of the chest with nasal flaring and inter costal retractions. After stabilization, advanced diagnostic tests, including X-rays and CT scans, confirmed the diagnosis of congenital lobar emphysema affecting the right middle lobe. The baby was promptly admitted to intensive care for immediate oxygen therapy. Swift action was taken, transferring the baby to the operating room for life-saving surgery. The procedure was done with postero lateral thoracotomy.

Dr. Dhiraj Balaji, Senior Consultant, Paediatric Surgery, Aster RV Hospital, who operated on the baby explained, “We had to face Anaesthetic challenges while operating the 27 day old baby. The baby could collapse the moment relaxant was given. It was like an awake anaesthesia. After sedation, the chest of the child was opened, the Lung was delivered out and the baby improved instantly, anaesthetic relaxants were given later to bring baby to normalcy. The challenge with Neo Nat is, the lungs are very fragile. It was stuck with other parts of the body and major blood vessels, including the heart and other organs. This was inside the chest cavity, behind the ribcage. The lobe which was enlarged due to the defect was more than the size of both the lungs like a balloon. Surgery had to be done with utmost care and precaution. Post operation, the baby was shifted to NICU. It was on ventilator at the NICU on day 2, next day it was extubated, it was later shifted to the ward and was discharged on the 5th day.

This case was one in 10,000, a rare and intricate procedure. The defect Middle lobe of the right lung was rarest as the Commonest occurs on the left upper lobe. If the parents had delayed for another two to three days, it would not have survived. With the removal of one part of the lobe, the child can still perform normal activities and Vital capacity is not affected in the long run too”, the doctor concluded.

The baby’s father, who runs his own business at Ananthapur said, that it was a timely intervention. “When we took the baby to the hospital, it had a bloated stomach. Doctors said that the air was going inside but not coming out. Air was getting trapped causing compression of the surrounding lung. The heart was pushed to one side and the opposite lung was also compromised. The baby’s oxygenation was completely deprived. Baby was in distress and hunger for oxygen. CT Scan and X-Ray helped diagnose the right side lung middle lobe expanding like a balloon making it difficult for the baby to breathe.”

The baby was immediately shifted to ICU and oxygenation was given. Once the oxygenation to the brain improved, baby was operated upon. The middle lobe of the right lung was defective. After taking oxygen inside, it was unable to throw the oxygen out causing it to bloat. A very carefully guided surgery gave instant relief to the baby. The middle lobe of the right lung was removed and the baby’s condition improved instantly.

The baby is now thriving well with good weight gain when examined on a follow-up routine check-up visit to the hospital. The successful intervention not only saved the baby’s life but also ensured the possibility of a healthy and active life ahead. The baby’s vital capacity and ability to perform regular activities remain unaffected, ensuring a promising future” the doctors informed.

Bangalore Doctors Successfully Treat Complex Multiple Fistulae in Ano of Somaliland Man

Bengaluru: A team of medical professionals from Ramakrishna Super Speciality Hospital at Jayanagar has successfully treated chronic multiple fistulae-in-ano of a 39-year-old person from Somaliland, Africa. Ahmed (name changed), the patient who is a health inspector, was referred to the hospital for treatment by the Head of the Department of a reputed medical college.

Ahmed’s was a complex case involving multiple fistulae-in-ano (six fistulae, comprising high and low types), which led to recurrent pus discharge, pain, and compromised quality of life over a prolonged period. The patient had a history of diabetes and suffered from recurring perianal abscesses over the past year. He had been operated on at his hometown by surgeons with no permanent relief in sight.

Soon after Ahmed was brought to and admitted at the Ramakrishna Hospital, a team of doctors and the Minimally Invasive Proctology department conducted a thorough diagnosis, which involved performing a Transrectal ultrasound and MRI fistulogram to assess the extent of the fistulae. A limited colonoscopy was also carried out to eliminate the possibility of inflammatory bowel disease and tuberculosis.

A fistula is characterized by an abnormal connection between the skin and mucosal surface, causing a recurrent discharge of pus and discomfort. As for the anal region, it specifically denotes an anomalous link between the anal canal and the surrounding skin, leading to persistent pain and discharge.”

During the initial surgery, fistulectomies were performed using a laser for the low fistulae, along with the protection of the anal sphincter complex. For the high fistula, a Seton application was employed. Seton is a thread-like material used to maintain openings in the body, facilitating drainage and promoting healing in conditions like abscesses or fistulae, explained Dr Premnath.

Following a smooth post-operative period of two months, Ahmed underwent a second surgery for the high fistula, involving the removal of the Seton. Subsequently, post-procedure observation for three months revealed successful healing and the patient recovered well, returning to his home country hale and hearty.

“When ignored, a fistula can increasingly complicate over time, affecting the individual and quality of life. The more intricate the fistula, the more complex the surgical procedure, heightening the risk of harm to the anal sphincter. Consequently, early diagnosis and treatment are crucial for effectively managing a fistula. This case illustrates a staged approach to complex fistulae, emphasizing the significance of protecting the anal sphincter,” summed up Dr Premnath.
